Application Package for The University College Faculty Service Award

 

Submit to [email protected] by April 1:

  1. Current curriculum vitae
  2. A brief, one-page, single-spaced (no more than 500 words) personal statement summarizing:
    1.  Service to the campus
    2.  Service to the college
    3. Service to the University
    4.  Service to the discipline and profession
    5.  Service to society as a representative of the University
    6.  Service to the student body through advising and student affairs activities  
  3. Request for three people to write brief, one-page, single-spaced letters (no more than 500 words) in support of their nominations. These letters should be sent directly to [email protected].
